MASC Upstate Waiver Form – Onam 2025

Event: Onam 2025                                                       Date: Sept 06th, 2025.

Location: Vedic Center, 520 Bethel Dr, Mauldin, SC- 29620.               Time: 3PM to 9PM

Malayalee Association of South Carolina – Upstate (hereby referred as MASC), is happy to welcome you (hereby referred as participant) for the event Onam 2025. By registering for this event conducted by MASC upstate, participant understands that this activity is voluntary on their part.

Participant acknowledge that he/she is aware that there are risks to them, of exposure to directly or indirectly arising out of, contributed to, by, or resulting from: An outbreak of any and all communicable disease, including but not limited to, the virus “severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2)”, which is responsible for Coronavirus Disease (COVID-19) and/or any mutation or variation thereof.  In consideration of having the opportunity to participate and in acknowledging that he/she is aware of and willing to assume the risks associated with this activity, participant hereby voluntarily agree to waive, hold harmless and indemnify MASC and its members, volunteers and employees from any and all claims, demands, damages and causes of action of any nature whatsoever arising out of ordinary negligence which they(participants) or their assigns may have against them for, on account of, or by reason of my participation.

Participants agree to waive and relinquish all claims that they may have against MASC Upstate and its members or volunteers or coordinators of the event Onam 2025.

Participants further agree to indemnify and hold harmless and defend the MASC Upstate and its coordinators and members from any and all claims resulting from injuries, including damages to his/her belongings or any losses that may cause due to the event participation.

If any of the committee member registered on behalf of the participant, MASC Upstate assumes that the participant is aware of the waiver, and hereby adhere to the waiver. Participant cannot hold MASC Upstate or any of the committee member responsible in such cases.

Please be advised that, food served during the event may contain milk, wheat, soybean, peanuts, tree nuts and other dairy products. Please ask a committee member or volunteer, about the ingredients used in the meal, before consuming. Participants further agree to release MASC Upstate and its committee members/volunteers from any liability with regard to possible spoilage or food-borne illness from the event site.
